Serena Jaskolka, MS, CCC-SLP, CDP, is a speech-language pathologist with clinical experience across skilled nursing, assisted living, memory care, and acute care settings. Her clinical work focuses on dysphagia management in post-acute care, where she also provides FEES to expand access to instrumental swallowing assessment in skilled nursing facilities.
Serena is the founder of SNF Toolkit, an educational resource designed to support SLPs working in skilled nursing facilities through practical, evidence-informed content on clinical practice, reimbursement, regulations, and navigating the SNF setting.
Her work is grounded in a commitment to patient-centered care, with an emphasis on supporting individuals in making informed choices that align with their values and allow for meaningful participation in daily life.
